Frustrated with Master-Builders Naples's lack of response

I am writing regarding order #ORD-5171-SJV-8015 placed with Master-Builders Naples on January 2, 1994. I paid $532.19 for plan, but when it arrived, I discovered arrived significantly late. I've made numerous attempts to contact customer service, but Rubie O. and other representatives have not been helpful in resolving this matter.
