Hidden fees discovered from Homes of Prestige

I am writing regarding order #ORD-5954-PRQ-7373 placed with Homes of Prestige on December 4, 2003. I paid $647.20 for plan, but when it arrived, I discovered arrived significantly late. I've made numerous attempts to contact customer service, but Abelardo H. and other representatives have not been helpful in resolving this matter.
