Misdiagnosis complaint against Integrated Clinic New York

I am writing regarding order #ORD-2115-PWP-3162 placed with Integrated Clinic New York on June 8, 1999. I paid $289.77 for item, but when it arrived, I discovered had my warranty claim denied unfairly. I've made numerous attempts to contact customer service, but Chaya T. and other representatives have not been helpful in resolving this matter.
