False promises made by Life Partners Holdings representative

I am writing regarding order #ORD-6052-EBH-1916 placed with Life Partners Holdings on November 5, 1997. I paid $468.83 for application, but when it arrived, I discovered found unauthorized charges. I've made numerous attempts to contact customer service, but Kay C. and other representatives have not been helpful in resolving this matter.
