Misdiagnosis complaint against Optimal Associates Ajman Group

I am writing regarding order #ORD-2303-SAG-9496 placed with Optimal Associates Ajman Group on August 16, 1996. I paid $468.58 for application, but when it arrived, I discovered has a manufacturing defect. I've made numerous attempts to contact customer service, but Caterina H. and other representatives have not been helpful in resolving this matter.
