Hidden fees discovered from PrimeHealing

I am writing regarding order #ORD-9600-EVR-1734 placed with PrimeHealing on September 29, 2015. I paid $569.46 for equipment, but when it arrived, I discovered experienced frequent disconnections. I've made numerous attempts to contact customer service, but Coby I. and other representatives have not been helpful in resolving this matter.
