Hidden fees discovered from Remedy Solutions

I am writing regarding order #ORD-2197-RHD-4482 placed with Remedy Solutions on July 3, 1991. I paid $63.25 for accessory, but when it arrived, I discovered stopped working after two weeks. I've made numerous attempts to contact customer service, but Jasen G. and other representatives have not been helpful in resolving this matter.
