Billing dispute with Counsel Services of Fair Holdings

I am writing regarding order #ORD-8364-XDK-3501 placed with Counsel Services of Fair Holdings on February 9, 1995. I paid $375.12 for plan, but when it arrived, I discovered was denied a refund. I've made numerous attempts to contact customer service, but Esther D. and other representatives have not been helpful in resolving this matter.
