Hidden fees discovered from Flavors Co

I am writing regarding order #ORD-1607-NLS-8737 placed with Flavors Co on May 5, 2017. I paid $83.73 for application, but when it arrived, I discovered stopped working after two weeks. I've made numerous attempts to contact customer service, but Dan H. and other representatives have not been helpful in resolving this matter.
