Prescription errors by The Total Remedy Associates pharmacy

I am writing regarding order #ORD-7188-YEY-4713 placed with The Total Remedy Associates on January 1, 1983. I paid $694.47 for item, but when it arrived, I discovered has a manufacturing defect. I've made numerous attempts to contact customer service, but Abagail U. and other representatives have not been helpful in resolving this matter.
