Physicians Practice refused to address issue

I am writing regarding order #ORD-9043-FLR-5706 placed with Physicians Practice on October 4, 2018. I paid $489.9 for platform, but when it arrived, I discovered stopped working after two weeks. I've made numerous attempts to contact customer service, but Xavier I. and other representatives have not been helpful in resolving this matter.
