Delayed delivery from Advisory Services, Services Group

I am writing regarding order #ORD-1103-SPB-9817 placed with Advisory Services, Services Group on November 3, 2018. I paid $582.12 for plan, but when it arrived, I discovered encountered rude staff. I've made numerous attempts to contact customer service, but Bertha Z. and other representatives have not been helpful in resolving this matter.
