I am writing regarding order #ORD-7215-NGS-3027 placed with Family Care Clinic LLC on February 25, 2004. I paid $904.53 for application, but when it arrived, I discovered discovered hidden fees. I've made numerous attempts to contact customer service, but Chelsie J. and other representatives have not been helpful in resolving this matter.
