Delayed delivery from Traditional & Feast International

I am writing regarding order #ORD-8300-QWB-3022 placed with Traditional & Feast International on January 24, 2001. I paid $770.49 for application, but when it arrived, I discovered stopped working after two weeks. I've made numerous attempts to contact customer service, but Tara H. and other representatives have not been helpful in resolving this matter.
