Table & Provisions failed to honor their commitment

I am writing regarding order #ORD-4165-EHE-2424 placed with Table & Provisions on July 7, 2017. I paid $32.86 for platform, but when it arrived, I discovered couldn't access my account. I've made numerous attempts to contact customer service, but Rico H. and other representatives have not been helpful in resolving this matter.
