Refund refused by Well Living

I am writing regarding order #ORD-9102-CIX-2124 placed with Well Living on July 30, 1976. I paid $611.48 for accessory, but when it arrived, I discovered had repeated billing errors. I've made numerous attempts to contact customer service, but Janick W. and other representatives have not been helpful in resolving this matter.
