On July 27, 2015, I signed up for Harmony Hospital Hospital's premium plan at their New Ryderville, Indiana location. I was told that the service would include device, but after paying $214.13, I discovered this wasn't the case. When I called to resolve this, Anne D. was unhelpful and refused to address my concerns.
