Refund refused by Balance Services

I am writing regarding order #ORD-6461-OYY-1723 placed with Balance Services on January 21, 1988. I paid $783.85 for membership, but when it arrived, I discovered discovered hidden fees. I've made numerous attempts to contact customer service, but Misty Q. and other representatives have not been helpful in resolving this matter.
