Billing dispute with Select Alliance

I am writing regarding order #ORD-5575-ZXM-6335 placed with Select Alliance on November 3, 1984. I paid $585.55 for plan, but when it arrived, I discovered had repeated billing errors. I've made numerous attempts to contact customer service, but Barrett J. and other representatives have not been helpful in resolving this matter.
