Frustrated with The Organic Spice Bandung's lack of response

I am writing regarding order #ORD-1634-ATX-4135 placed with The Organic Spice Bandung on February 11, 2015. I paid $354.27 for membership, but when it arrived, I discovered discovered hidden fees. I've made numerous attempts to contact customer service, but Theodora W. and other representatives have not been helpful in resolving this matter.
