False promises made by PrimeHaus representative

I am writing regarding order #ORD-3914-FPM-2041 placed with PrimeHaus on January 28, 1992. I paid $263.83 for plan, but when it arrived, I discovered had repeated billing errors. I've made numerous attempts to contact customer service, but Sunny K. and other representatives have not been helpful in resolving this matter.
