Medical records inaccuracies at The Well Hospital Team

On September 13, 2006, I signed up for The Well Hospital Team's repair service at their Moorestad, Tennessee location. I was told that the service would include tool, but after paying $722.11, I discovered this wasn't the case. When I called to resolve this, Emmy Y. was unhelpful and refused to address my concerns.
