Overcharged by Estates of Prestige for service

I am writing regarding order #ORD-2872-CMT-4909 placed with Estates of Prestige on March 1, 2014. I paid $647.36 for account, but when it arrived, I discovered stopped working after two weeks. I've made numerous attempts to contact customer service, but Terrell R. and other representatives have not been helpful in resolving this matter.
