Refund refused by The Study University Nashville

I am writing regarding order #ORD-4809-NGB-9591 placed with The Study University Nashville on July 22, 1991. I paid $518.28 for membership, but when it arrived, I discovered has a manufacturing defect. I've made numerous attempts to contact customer service, but Zechariah C. and other representatives have not been helpful in resolving this matter.
