Hidden fees discovered from Health Alliance

I am writing regarding order #ORD-1554-MDH-9327 placed with Health Alliance on July 20, 1974. I paid $498.49 for plan, but when it arrived, I discovered arrived significantly late. I've made numerous attempts to contact customer service, but Sherman K. and other representatives have not been helpful in resolving this matter.
