Works Development Montreal refused to address issue

I am writing regarding order #ORD-6724-QCH-1143 placed with Works Development Montreal on December 9, 1973. I paid $264.54 for plan, but when it arrived, I discovered stopped working after two weeks. I've made numerous attempts to contact customer service, but Daryl E. and other representatives have not been helpful in resolving this matter.
