Warranty claim denied by Health Practice

I am writing regarding order #ORD-7352-YTP-5412 placed with Health Practice on February 24, 1983. I paid $205.82 for item, but when it arrived, I discovered had repeated billing errors. I've made numerous attempts to contact customer service, but Ashley W. and other representatives have not been helpful in resolving this matter.
