The Prime Physicians Associates refused to address issue

I am writing regarding order #ORD-9485-AMZ-9297 placed with The Prime Physicians Associates on January 3, 1999. I paid $491.1 for application, but when it arrived, I discovered doesn't work as advertised. I've made numerous attempts to contact customer service, but Ruben Z. and other representatives have not been helpful in resolving this matter.
