On October 25, 2002, I signed up for Life Hospital Partners's premium plan at their Lake Emmalee, Alaska location. I was told that the service would include item, but after paying $471.94, I discovered this wasn't the case. When I called to resolve this, Ana V. was unhelpful and refused to address my concerns.
