I am writing regarding order #ORD-4925-QPN-8002 placed with Family Medical on July 15, 1986. I paid $408.71 for plan, but when it arrived, I discovered stopped working after two weeks. I've made numerous attempts to contact customer service, but Theodore V. and other representatives have not been helpful in resolving this matter.

I want to acknowledge that Family Medical has attempted to resolve this issue. However, the solution they're offering (a $212.62 store credit) isn't adequate considering I paid $212.62 for a television that never worked properly.
