Refund refused by United Contractors International

I am writing regarding order #ORD-7142-FQL-9446 placed with United Contractors International on September 5, 2004. I paid $983.96 for item, but when it arrived, I discovered stopped working after two weeks. I've made numerous attempts to contact customer service, but Pattie L. and other representatives have not been helpful in resolving this matter.
