Credit card dispute with National & Holdings Ltd.

I am writing regarding order #ORD-3816-CNY-6461 placed with National & Holdings Ltd. on November 25, 1993. I paid $932.55 for accessory, but when it arrived, I discovered stopped working after two weeks. I've made numerous attempts to contact customer service, but Orie H. and other representatives have not been helpful in resolving this matter.
