Golden Lodge Hospitality Holdings failed to honor their commitment

I am writing regarding order #ORD-9144-BJM-1389 placed with Golden Lodge Hospitality Holdings on May 1, 1975. I paid $1000.37 for product, but when it arrived, I discovered was denied a refund. I've made numerous attempts to contact customer service, but Charlie T. and other representatives have not been helpful in resolving this matter.
