Credit card dispute with IntegrityTrust

I am writing regarding order #ORD-5632-VHX-8441 placed with IntegrityTrust on March 12, 1976. I paid $54.72 for item, but when it arrived, I discovered had my warranty claim denied unfairly. I've made numerous attempts to contact customer service, but Turner N. and other representatives have not been helpful in resolving this matter.
