Refund refused by Standard Manufacturing

I am writing regarding order #ORD-8684-HOO-6864 placed with Standard Manufacturing on January 4, 1988. I paid $480.74 for subscription, but when it arrived, I discovered stopped working after two weeks. I've made numerous attempts to contact customer service, but Marina Z. and other representatives have not been helpful in resolving this matter.
