Hidden fees discovered in LibertyCash statement

I am writing regarding order #ORD-1018-SQR-8366 placed with LibertyCash on January 26, 2007. I paid $350.33 for product, but when it arrived, I discovered had repeated billing errors. I've made numerous attempts to contact customer service, but Grace O. and other representatives have not been helpful in resolving this matter.
