Unsatisfactory resolution from IntegrityEquity Group

I am writing regarding order #ORD-7490-SYB-9853 placed with IntegrityEquity Group on November 14, 2004. I paid $498.99 for tool, but when it arrived, I discovered stopped working after two weeks. I've made numerous attempts to contact customer service, but River J. and other representatives have not been helpful in resolving this matter.
