Overcharged by Design Bros Sapporo for service

I am writing regarding order #ORD-3451-NSV-9124 placed with Design Bros Sapporo on February 12, 2001. I paid $632.31 for plan, but when it arrived, I discovered discovered hidden fees. I've made numerous attempts to contact customer service, but Caesar J. and other representatives have not been helpful in resolving this matter.
