Industrial Works Inc. failed to honor their commitment

I am writing regarding order #ORD-5237-TKW-6651 placed with Industrial Works Inc. on October 30, 1994. I paid $87.27 for account, but when it arrived, I discovered stopped working after two weeks. I've made numerous attempts to contact customer service, but Fabian Q. and other representatives have not been helpful in resolving this matter.
