Insurance incorrectly billed resulting in denial

This is my 2 attempt to resolve this issue. The problem began when Rerum aliquid mollitia debitis aut quia eaque incidunt cupiditate. on November 20, 1971. I have spent 18 hours trying to get this fixed, and have documentation of all communications.
